
Sage AWARD ENTRY FORM
Thank you for your interest in applying for the second annual Sage Award. If you have questions regarding the award or application, please contact Pat Bowling at 336/881-1006, or Sageaward@ahfa.us.
There are THREE PARTS to a Sage Award Entry:
Part 1: Cover Page
Please provide all company contact information requested. The cover page is the ONLY place your company name should appear. Upon receipt of the completed Cover Page, Executive Summary and Detail Questionnaire, your entry will be assigned a code number, and the cover page will then be separated from the rest of your entry until the judging is complete.
Part 2: Executive Summary
The executive summary will be used in the first stage of judging to determine which entries will go on to the second and final round of judging. Do not write your company name or brand name anywhere in this document. Do not include confidential information in the summary. If you are a user of bio-based foam, do NOT state what brand of foam you use.
The summary should be 750 words or less and should begin by briefly describing your company’s primary business. The balance of the summary should provide an overview of your company’s sustainable business practices and social responsibility activities. Please use the “word count” tool (typically found in the “tools” menu of Microsoft Word) to check your length.
Part 3: Detailed Questionnaire
The questionnaire will provide the detail necessary for judges to complete their final selection of the 2009 Sage Award winner. Again, do not write your company name or brand name anywhere in this document. Do not include confidential information. If you are a user of bio-based foam, do NOT state what brand of foam you use. Read the questions carefully, and include only the information requested. You will have an opportunity at the end to provide additional commentary not covered by the specific questions.
All three documents must be received electronically via email by midnight Eastern Standard Time on Friday, September 4, 2009.
